New Patient Registration

Patients wishing to seek registration with the practice can collect a new patient registration pack by calling into the surgery.  Registration forms must be fully completed and you need to provide all the necessary registration documents required by the Business Services Organisation Patient Registration Team when submitting your application. Anyone who is accepted as a new patient onto the practice list must complete a new patient health declaration form. This is to establish any details of significant medical history, repeat medication required and to carry out a health check. New patients who require repeat medication will be asked for consent to allow the practice to contact their previous GP or to access the NIECR system for details of their repeat medication. If the patient provides a mobile telephone number, they will also be asked to declare consent for the use of SMS text messaging in appropriate circumstances.

It is the doctor’s decision as to whether a patient will be accepted onto the list or not.
